* fix(issues): load every Gantt page when the server caps page size

The Gantt fetch asked for 500 issues per page, but ListIssues clamps
limit to 100. The 100-issue first page looked like a short final page,
so projects with more than 100 scheduled issues lost every bar after
the first 100.

Request pages of 100 and advance the offset by the number of issues
actually returned.

Fixes #9572

* docs(issues): clarify Gantt page limit must match server cap
