Since all-in Cloudflare means TanStack Start only, add Step 2 to ask users
whether to remove Next.js and Nuxt.js apps before proceeding with CF config.
Reuses existing framework-cleanup.md reference from tinyship-setup skill.

When user chooses PostgreSQL, the agent now:
1. Runs pg_isready to check if PG is already running locally
2. If running, automatically creates a tinyship database via createdb
3. Auto-detects the connection string and confirms with the user
4. Falls back to Docker or manual input only when auto-detect fails
This avoids blocking the setup flow when the user has a local PG
instance but hasn't prepared a connection string in advance.

- Add "STOP — You MUST ask the user" directives at all decision points
(database choice, framework choice, cleanup confirmation) to prevent
agents from making choices on behalf of the user
- Restructure flow as conversational wizard with clear option tables
- Add missing `mkdir -p data` step before SQLite db:push to prevent
"directory does not exist" error
- Improve framework-cleanup reference with simplified script renaming
guidance and docker-compose profile removal notes
